Starting April 1, 2021 this new form is required for new service unlocks when the appliance installation manual requires the appliance be commissioned or setup.
The form should also be used for construction heat unlocks, where the gas meter has already been unlocked for a portable heater. We are allowed to activate the furnace for construction heat in these cases.

Ahhh this makes sense now. Union Gas has been on a rampage with us lately. They just (last month) wrote up a building we did 5 installs at mid-last year for not having "startup pages" attached. They must have meant this, although not required at that install.
Just got more info on this. April 1st - May 31 is the grace / phase in period for this required document on all appliances needing commissioning per the manufacturer's instructions. After June 1st it will be required to be affix to all equipment or Btag (or no final) will be given.
Fill out section A, and check all the boxes. Unless the appliance used for construction / finishing. Then fill out section B as well.
Will they supply carbon copies of this form? I see at the bottom multiple copies are needed.
Enbridge will just want to see the one form at the appliance.
For single page forms they say,
"The download will be a single copy form and it is recommended that the TSSA Certificate Holder take an image of the form upon completion for their records."

Enbridge has also had online training for this. I believe they have more online training March 30 and 31. I will post that info on Monday.

Apparently you can complete this form without commissioning the appliance, as a promise to return to do it, and they will not issue a warning tag.
Yes, promise to return. I think check off box #1 of my memory serves me right!
